The City of Pueblo is continuing to make improvements to Lake Minnequa Veterans Memorial Park located in Pueblo, CO. The current phase of the improvement plan includes adding a building to house restrooms, a concession stand, and storage. The preferred building size is 1,000 sq. ft. plus an additional roof area to cover a picnic table patio. Proposers should have experience with engineering, design, and manufacture/construction of modular buildings which meet the project needs. Only proposers with adequate experience and able to meet HUD, State of Colorado, and Pueblo Regional Building requirements will be considered.

The City of Pueblo is continuing to make improvements to Lake Minnequa Veterans Memorial Park located in Pueblo,

and storage. The preferred building size is 1,000 sq. ft. plus an additional roof area to cover a picnic table patio.
Proposers should have experience with engineering, design, and manufacture/construction of modular buildings which
meet the project needs. Only proposers with adequate experience and able to meet HUD, State of Colorado, and
Pueblo Regional Building requirements will be considered.
This Request for Qualifications (RFQ) will be evaluated on the criteria listed in Section 3 of this document. A scored
evaluation will be conducted for each Statement of Qualifications (SOQ/Submittal). Proposed project cost will not
be included in the evaluation and Proposers MUST NOT include cost or fee information within their Submittal.
Submittals received with cost or fee information may be considered non-responsive and rejected.
Please be advised that electronic submissions (i.e. fax, emails, etc.) will not be accepted as a sealed Submittal.
Proposers are urged to read the attached solicitation documents thoroughly before submitting an SOQ.
The City reserves the right to reject any and all Submittals for any or all items covered in the Request for
Qualifications, to waive informalities or defects in the RFQ documents and/or Submittals or to accept any submittal
as it shall deem to be in the best interest of the City.
Disclaimer
The City provides all non-construction solicitations for interested parties to download free of charge via the Rocky
Mountain E-Purchasing System (RMEPS) and on the City’s website, www.pueblo.us/purchasing. Proposers can also
choose to register with RMEPS to get notifications automatically emailed to them for a nominal annual fee.
Additionally, interested parties may visit the Purchasing Office during normal working hours and request hardcopies
of any current solicitation at the same cost allowed for CORA requests. If the Proposer cannot verify that the RFQ
documents were obtained from either of these two websites or our office, we cannot guarantee the validity of
the document and their Submittal may be rejected.

SECTION 1. SUBMITTAL FORMAT AND REQUIRED RESPONSES
The information set forth in the paragraphs below must be included with all Submittals. A total of 5 copies of the
complete Submittal including 3 hard copies bound and tabbed, 1 hard copy unbound and untabbed, and 1 copy
on a USB/flash drive must be included. Do NOT email or fax copies.
Responses shall be considered technical SOQs and shall be incorporated in award documents as deemed appropriate
by the City. Please attach your responses to these items to the Submittal Cover Sheet and Signature Page. Failure of
Proposers to respond to any of the following technical submittal requirements may be grounds for considering a
Submittal non-responsive.
Note: Proposed project cost will not be included in this evaluation and Submitters MUST NOT include cost or
fee information within their submission. Submittals received with cost or fee information may be considered
non-responsive and rejected.
1.1 Submittal Cover Sheet
The Submittal Cover Sheet and Signature Page (pages 2-3) must be completed and returned with the Submittal. Failure
to return the signed Cover Sheet is grounds for the City to reject a Submittal.
1.2 Certificate of Good Standing
Proposer must attach a Certificate of Good Standing from the state wherein Proposer was organized. For Colorado
the certificate can be downloaded at: https://www.sos.state.co.us/biz/BusinessEntityCriteria.do.
1.3 Table of Contents
The Table of Contents must indicate the material included in the Submittal by section and page number. A Submittal's
table of contents should mirror this section of the Request for Qualifications and must include all the items set forth
in this section of the RFQ.
1.4 Submittal Summary
A letter providing the following information must be submitted with the SOQ. The letter must include:
The names of the persons who are authorized to make representations on behalf of the Proposer (include
their titles, addresses, fax number, e-mail addresses and telephone numbers).
A statement that the individual who signs the transmittal letter is authorized to contractually bind the
Proposer to contract with the City of Pueblo.
1.5 Disclosures
If applicable, disclose any professional or personal financial interest, which could be a possible conflict of interest in
providing products and services to the City. If not applicable, please make a brief statement indicating that.
1.6 Statement of Qualifications
Identify the Proposer’s company name, address, phone and primary contact person. Provide a brief history
and overview of the company and its organizational structure.
Identify the prime Consultant/Contractor and all sub-contractors, including their roles and responsibilities
in the project.
Describe Consultant/Contractor and staff’s professional qualifications, availability for this project, and
experience on similar projects (similar in size and scope). Only individuals who will actually work on this
project should be identified.
Describe projects of similar size/scope performed within the last five (5) years demonstrating the
Consultant’s ability to meet schedule deadlines without delays, cost escalations, and vendor claims.

Provide references for at least three (3) previous projects. Reference must include company name, location
of project, address, company representative/contact person, contact email, and contact phone number.
If available, provide a copy of your company’s Purchase Agreement. City may utilize Contractor’s
Purchase Agreement form with revisions/alterations as deemed necessary by the City’s Law Department.
Provide information on setup and installation, if available. Do not include pricing, only provide details
and process information. Prices will be requested in the Request for Proposal to follow this process.
1.7 Design Review
Provide a minimum of three (3) preliminary designs able to meet the Minimum Specifications outlined in
Section 2. Include floor plans, elevations, and perspectives for a custom designed modular building.
The preliminary design shall identify available interior and exterior materials, finishes and colors. The
desired exterior finishes are steel / metal, natural wood, stone siding (minimal stucco may be allowed), and
metal roofing.
The overall design should closely resemble the existing shelter at the Park’s basketball courts.
Additionally, the structure should incorporate a mixture of materials as shown in the attached
examples/concepts of the desired architectural theme. (Refer to Appendix C)
Note: Please do not provide more than six (6) floorplans/variations. City will make suggestions/revisions
prior to requesting a price quote in the Request for Proposal (next phase of this project).
1.8 Project Approach/Time Frame
Provide details outlining the normal processes including order, licenses/plan approvals,
manufacture/construction, delivery, installation, etc.
The Submittal shall include the number of days needed for the Contractor to complete the build of the
selected modular unit.
Include anticipated delivery dates, normal setup/installation expectations, etc. Please note if setup and
installation shall be performed by modular manufacturer or alternate contractor.
SECTION 2. MINIMUM SPECIFICATIONS
2.1 Overview
The City of Pueblo is considering modular construction companies of the highest quality to supply, deliver, and install
a custom-designed 1,000 sq. ft. modular building for restrooms, concessions, and storage with a roof overhang for a
covered patio.
2.2 Location
The building shall be located at Lake Minnequa Veterans Memorial Park located at 3001 Palmer Avenue, Pueblo, CO
81004.
2.3 General Specifications
The building will be of modular construction, approximately 1,000 sq. ft. with restrooms, storage and concession areas
and a roof overhang that will cover a seating area in the plaza.
2.3.1 Concession Area (approximately 250 sq. ft.)
Area shall provide adequate room for warming/heating, no cooking
Provide access to the exterior and a separate storage room (approximately 50 sq. ft.)
Serving counter with service door and canopy for customers
Installed floor sink, three (3) compartment sinks, and hand-washing sink
