RFP #23-11 - RFP 23-10 Older Adult Health and Aging in Place Services

The County of San Joaquin is soliciting Request for Proposals (RFP) for its HUMAN SERVICES AGENCY DEPARTMENT, AGING & COMMUNITY SERVICE DIVISION Department/Division. The Older Americans Act (OAA) was enacted by Congress in 1965. This Act, and its subsequent amendments, has the goal of maintaining or improving the physical, social and psychological well-being of persons aged sixty and older. This goal is partially carried out by the allocation of Federal Funds. These Federal funds are allocated to states and, subsequently, to local Planning and Service areas who in turn award contracts for service delivery to community organizations. Furthermore, the Modernizing Older Californians Act (OCA) builds on the existing Older Americans Act. Other sources of funding may be used. The San Joaquin County Department of Aging & Community Services within the Human Services Agency (HSA) is the designated Area Agency on Aging for Planning and Service Area 11 (PSA 11) which includes all of San Joaquin County. The mission is to coordinate the development of a Community Based System of Care (CBSC)1 to older persons living in San Joaquin County so they may retain as much independence as possible. The San Joaquin County Department of Aging is requesting proposals to provide older adult (60 years and over) services to the targeted population in San Joaquin County. The contract will be for an initial period beginning date of execution and ending June 30, 2024, with an option of one (1) year extensions over the next three (3) years, based on satisfactory performance and funding availability. **OPTIONAL LETTER OF INTENT TO RESPOND** Prospective applicants must submit Exhibit C - Letter of Intent to Cecilia Silva at crsilva@sjgov.org by 4:00 pm Tuesday, November 07, 2023 .
