Subject: Request for Quotation number RFQ 19KU2026Q0012 - RESTROOMS & LACTATION ROOM UPGRADE PROJECT

Enclosed is a Request for Quotations (RFQ) for the Chancery Annex Restroom, Lactation RM & Locker RMs Renovation. If you would like to submit a quotation, follow all solicitation instructions, and complete the required portions of the attached document.

The U.S. Government intends to award a contract/purchase order to the responsible company submitting a technically acceptable offer at the lowest price. We intend to award a contract based on initial quotations, without holding discussions, although we may hold discussions with companies in the competitive range if there is a need to do so.

Note: Prospective offerors are strongly encouraged, though not required, to register in the System for Award Management (SAM) at https://www.sam.gov.

• Offerors with an active SAM registration need only provide their Unique Entity Identifier (UEI) number and are not required to complete the information 52.204-90 since relevant information is found in their SAM registration.


• Offerors without an active SAM registration must complete FAR 52.204-90, Offeror Identification, in Section 5, Representations and Certifications. This information must be updated annually if option years are included in the solicitation or award.

Offerors are strongly encouraged to maintain an active SAM account

For a quotation to be considered, you must also complete and submit the following:

1. SF-1442 (14 through 20c)


2. Section A, Price


3. Section L, Representations, certifications, and other statements of offerors or quoters


4. Information and documentation required by Section J, Attachment A, and all other applicable provisions of this solicitation.


5. Proof of SAM Registration / Unique Entity ID, or if not SAM registered, complete FAR 52.204-90, Offeror Identification,

Failure to follow these instructions will render the offer non-compliant and ineligible for further consideration

The Purpsose of this amendment 0002 is to:

1. Post Questions and Answers; and

2. Attachment A – Statement of Work is hereby deleted in its entirety and replaced with the revised Attachment A – Statement of Work, attached to this amendment.. The revised Attachment A incorporates applicable technical clarifications and revisions resulting from the Government's responses to offeror questions, including, but not limited to, clarifications concerning room dimensions, drywall and moisture-resistant material requirements, tile and waterproofing requirements, counters and sinks, plumbing and drainage, shower and hand-sprayer requirements, ABA/ADA requirements, material and product submittals, project documentation, safety plans, warranty requirements, and other technical requirements addressed in the Questions and Answers.

3. Offerors shall prepare their quotations based on the solicitation as amended, including the revised Attachment A.

All other terms and conditions of the solicitation remain unchanged.
