STATEMENT OF WORK (SOW)

Veterans Benefit Administration
Fort Walton Beach Veterans Readiness and Employment Office (VR&E)

SCOPE

The purpose of this procurement document is for delivery and installation services to fulfill the Veterans Benefits Administration (VBA) Fort Walton Beach, FL VR&E space requirements at 1046 Mar Walt Drive, 3rd Floor, Fort Walton Beach, FL 32547. Please review the following attachments that are critical to supporting this SOW:

Attachment Two VRE Floorplan and Specifications

The Contractor shall provide delivery and installation services of selected furniture items by the government. Furniture shall be picked up from 345 Perryhill Road, Montgomery, AL and delivered to 1046 Mar Walt Dr, Fort Walton Beach, FL 32547.

II. PERIOD OF PERFORMANCE
The furniture installation shall commence after delivery of the furniture. Delivery of furniture shall be within 10 days of notice to proceed. The offeror shall complete installation in a maximum of 5 business days; all deliveries and installation to be performed during normal working hours (8:00am 4:00pm CST); all shipping and handling charges shall be paid by the offeror. Responsibility and liability for loss or damage shall remain with offeror until final inspection and acceptance by VBA.
III. SPECIFICATIONS

1. PLACE OF PERFORMANCE:

All delivery and Installation shall occur at the following address:

Fort Walton Beach, FL 32547

2. PERFORMANCE OF WORK ON GOVERNMENT PREMISES:
Current rules and regulations applicable to the premises, where the work shall be performed, shall apply to the contractor and its employees while working on the premises. These regulations include but are not limited to, presenting valid identification for entrance, smoking restriction and any safety procedures as outlined in the site regulations. All work is to be coordinated with VBA. The VBA primary Point of Contact (POC) will be Richie Adams, who may be reached at 334-213-3391 or Darrell Bradford who may be reached at 334-213-3377.

3. DEMOLITION AND REMOVAL: The successful contractor shall remove any debris and packing materials from the new furniture.

IV. INSTALLATION:

The furniture installation shall be conducted in 1 phase (one 5-day work week). As the project is discussed with the successful Offeror, the installation schedule shall be developed with VBA and the Lessor. Installation shall be in accordance with the above installation duration schedule.

In order to meet this timeframe, the Offeror shall be required to plan an installation schedule, which meets the completion date listed herein.

The Offeror shall coordinate all deliveries and installation times with the VBA Project Manager.

Reference attachment A: Typicals and design intent floor plan for installation purposes. Field verification shall be performed upon contract award.

Completion of the installation requirement shall require the following minimal tasks:
Provide on-site management, coordination and technical assistance by a project manager working with the Government representative throughout the installation phase.
Meet all incoming delivery vehicles at the building entrance, unload deliveries, check for damages, and move them to the desired areas. Areas shall be clear and carpeted. Trucks are not allowed to remain in the loading dock area.
Offeror shall provide two copies of each of the following:
Product Warranties
Copies of final drawings with part numbers
The Offeror shall arrange delivery to the VBA location in accordance with the Installation Schedule as developed by the VBA Project Manager.
The Offeror shall be responsible for any damage to the building, which occurs during delivery and installation. It is incumbent upon the successful offeror to identify any existing damage to the building by means of a Pre-Installation and Delivery Walk-Thru, and daily submissions of checklists to the designated agency representative on site. Proper measures (protective runners, pads, etc.) shall be utilized by the offeror to protect the building (elevators, walls, floors, etc.).
The Offeror is responsible for compliance of all products and installation to the General Specifications in the Schedule for Office Furniture (71-I) and any other GSA Schedule which products are offered, and with all applicable state, local and federal building codes, local ordinances, and regulations.
Clean up and Disposal of Waste Material --. Prior to completion of the work, the offeror shall remove from the vicinity of the work, all shipping containers, packing, rubbish, unused materials, and other like material, belonging to the Offeror or used under the offeror s direction. Waste material shall be disposed of by removal from the job site. Waste materials removed from the job site shall be disposed of in accordance with the above paragraph. The offeror shall make any necessary arrangements with private parties and with county officials pertinent to locations and regulations of such disposal. Any fees or charges to be paid for disposal of materials shall be paid by the offeror. It is at the offeror s option to furnish a dumpster on site to contain their debris. This dumpster shall be emptied regularly to ensure proper sanitation, if required. Use of the dumpster must be approved by the Lessor.
At least one week prior to the beginning of delivery and installation, the offeror s representative, supervisor, and members of the installation crew shall provide the following information to both the Contracting Officer and VBA Project Managers:
Company Name, Address, Telephone Number
Supervisor s Name and Social Security Number
Name and Social Security Number of each individual Installer
The Offeror shall be responsible for confining his crew and work to the designated project areas. Offeror shall also be responsible for ensuring that the installation personnel are identifiable.
THE OFFEROR IS ADVISED TO READ THE TERMS AND CONDITIONS OF THIS REQUEST FOR QUOTATIONS CAREFULLY.
Installation
Installation supervision shall be handled by an experienced on-site supervisor trained by the manufacturer or certified designee. The furniture system installers shall have the following qualifications: A minimum of two (2) years of successful experience in the installation of the proposed furniture; and Certification and ongoing factory training by the manufacturer.
Code Compliance: All products and installation shall comply with all state, local, and federal building codes, local ordinances, and regulations. The responsibility for determining and assuring compliance is that of the successful Bidder.
Building Protection - Reasonable building protection will be required to protect finishes from excessive damage. It is the responsibility of the installer to properly protect the building during installation. Walls around the doors should all be covered with corrugated board. All doorframes and doors should also be covered with special cardboard corner guards and corrugated board. All elevators are to be covered with blankets and cardboard. All carpet and tiled floors are to be covered with Masonite boarding throughout the main traffic areas, main lobbies and elevator lobbies. It is imperative that all surfaces be properly protected to eliminate damage fees.
Installation Hours: Conducted installation services during regular business hours (verifying with Lessor) to the maximum extent possible according to the schedule developed with the Lessor and VBA approval.
Verification of Shipments: Parts are to be checked at factory prior to shipping to ensure order is complete and that installation shall not be delayed because of missing parts or an incomplete delivery.
Repairs: Correct at Bidder expense or pay the Government to repair any building damage caused by the installation team, including damage to walls, ceiling, and floors. Restore the building to the condition it was in at the start of furniture installation. If the Bidder shall repair building damage, use to repair the damage that meet the environmental specifications required in the construction contract.
Touch-up all nicks and scratches on the furniture that occurs during installation.
Upon Completion of Installation
1. Protect all warranties by immediately correcting all omissions, mistakes, or damage.
2. Review the installation of the site with VBA representatives and prepare a punch list of any problems that shall be corrected.
3. Ensure installation conforms to the requirements of the Uniform Federal Accessibilities Standards [UFAS].
4. Unless directed otherwise by the VBA, remove from the project site any defective product or material that has been delivered to the site in error.
5. Correct all minor deficiencies and make all minor adjustments as required for life safety within seventy-two (72) hours after completing the installation. Replace all damaged or missing products by means that shall eliminate or minimize installation delay. Assume replacement costs, including communications, shipping, and installation.
6. Correct all other punch list items, to the satisfaction of the VBA within five (5) working days after completing the furniture installation, unless new product being ordered. Provide weekly updates.
7. Provide a professional installation that is square, plumb, and properly aligned with adjoining furniture.
Post Installation Services
1. Coordinate disposal activities with the building owner. Remove all packing materials, debris, and cartons from the job site daily.
2. Do not dispose of or discard recyclables, packing materials or trash in building-provided recycling or trash containers or dumpsters.
3. Furniture Cleaning: Clean and dust furniture surfaces inside and out with a spray cleaner upon completion of installation. Use only approved non-toxic cleaners.

Building Restrictions:

Installers need to coordinate with the VBA local POC to coordinate the installation. Advance notice needs to be given to the VA to coordinate with the building manager to access to the site. Delivery will need to take place during normal business hours and installation work to be performed during normal working hours (8:00am 4:30pm CST) unless coordinated differently with the local VA POC. Deliveries must be scheduled as to not disrupt building maintenance and functions. The facility is not equipped with a loading dock therefore the delivery vehicle shall be equipped with a lift gate.
