THIS IS A SOURCES SOUGHT NOTICE ONLY
This announcement is seeking information from the industry, which will be used for preliminary planning purposes for procurement. No proposals are being requested or accepted with this synopsis. THIS IS NOT A SOLICITATION FOR PROPOSALS AND NO CONTRACT SHALL BE AWARDED FROM THIS SYNOPSIS. THIS IS STRICTLY MARKET RESEARCH.
The Department of Veterans Affairs, Network Contracting Office 1, is seeking sources who have the capability for long term traumatic brain injury (TBI) and Acquired Brain Injury/Disorder (ABI) rehabilitative services for veterans with moderate to severe TBI/ABI in a state-licensed residential care setting for VAMC Manchester, New Hampshire.
Scope: The contractor will provide long-term services with qualified and annually trained staff to provide each Veteran with assistance with Activities of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL) in a residential setting as a State licensed community-based program designed exclusively for adults with brain injuries. These are qualified facilities by the state to provide Home and Community-Based Services under the Medicaid-Waiver program for Brain Injury.
Services:
The contractor shall provide personal assistance with Activities of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL), support and supervision, guidance, and treatment programs according to an individualized plan of care which is based upon the Veteran s personal goals. These goals are necessary to preserve and restore to the maximum extent possible; the physical, mental, and psychological functioning of the Veteran. In addition, the contractor will ensure that Veterans requiring medical care outside of the facility and are provided with medical treatment in facilities licensed by the State s Department of Health and Human Services (DHHS), as necessary, and based upon each resident's Plan of Care.

Staffed 24/7/365 by the Contractor s employees.
24 Hr. awake supervision and support, while residents are:

Community-based activities
Employment/or volunteer work
Medical/Health appointments
Recreational activities
Staff rehabilitation interventions will focus on:
Preserving and improving resident health and functional skills;
Behavioral management and adjustment;
Preserving and improving resident s positive social adaptation skills by increasing use of compensatory strategies and encouraging social and community engagement
Resident Medication and Durable Medical Equipment Supervision and Administration
Residents will self-medicate and implement durable medical equipment as prescribed with staff supervision and assistance as needed and as defined by New Hampshire Department of Health and Human Services (NHDHHS) Rules
Staff will educate residents on medication name, dosing and purpose as well as durable medical equipment proper usage and purpose
Staff will inform medical care providers when the resident s medical situation changes.
Medical Case Management
Oversight of medical care as needed
Confer as needed with Patient Aligned Care Team (PACT) team at Manchester VAMC as well as TBI/Polytrauma team.
Schedule, confirm and attend all resident medical appointments.
Confer with resident s guardian when applicable
Receive and deliver medical record information to and from facility
Follow up with sharing outcome of medical evaluations and changes with resident s guardian as necessary
Follow up on medication regimen and durable medical equipment as needed.
Trach care (no deep suctioning)
Feeding tubes (no constant delivery)
Minor skin integrity issues
Dysphasia related issues
Referral for emergent medical care typically provided by local hospitals or Urgent Care Centers
Care Coordination with medical and rehabilitation care provided by VISN 1 VAMCs
Nursing Services
Consulting Registered Nurse will be available on-call 24/7 for:
Staff Training and instruction on resident care
Staff Medication and Durable Medical Equipment Supervision training
Supervised Medication audits as described in NH DHHS rules
Consulting with medical providers as needed
Nursing evaluations for resident health
o NH Health Risk Screening Tool (HRST) and NH Resident Assessment Tool (RAT) done on site
Resident Services
Case Management identification and access for local community resources to meet resident needs and goals that enable community engagement
Behavior management and skill training to assist in community inclusion and social integration
Assistance with managing and completing ADL and IADL
Supervision with medication administration as well as the use of Durable Medical Equipment as prescribed
Life skills/training/Vocational opportunities
Job coaching/supervision or support
Program Transportation
Personal safety awareness and training
Social appropriateness training and practice
Meal planning and nutrition training and practice
On-site and in-community recreational activities
All interested firms must be registered in SAM (System for Award Management) to be eligible for the award of Government contracts. Interested parties responding to this Sources Sought request shall submit the following information at a minimum:
Company name and address point of contact with phone number and SAM UEI.
Documentation relating to capability of performance, training, and a current point of contact and phone number.
Socio-economic status to identify the business class as being a large business, SDVOSB, VOSB, small business, Woman Owned, etc.
Service Area
Any other pertinent company documentation prior to submission of offers to a solicitation.
Any SDVOSB firms responding to this Sources Sought Notice must also provide proof of registration in the SBA Vet Cert, prior to submission of offers to a solicitation.
This market research is for informational and planning purposes. This is not a solicitation and does not obligate the Government to issue a solicitation. Please be advised that the government will not pay any costs for responses and information submitted.

RESULTS INFORMATION: Respondents will not be notified of the results, and it is at the discretion of the Government to utilize results. Firms responding to this Sources Sought announcement, who fail to meet all requirements as stated above, and who fail to provide ALL the required information requested, will not be used to help the Government make the acquisition decision, which is the intent of this sources sought announcement.
