OFFEROR READ THIS INFORMATION FIRST BEFORE SUBMITTING AN OFFER
This is a combined synopsis/solicitation for commercial products and commercial services prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Products and Commercial Services, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, and a Request for Quote (RFQ) is being requested. Solicitation Number: 36C26323Q0991
The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2023-04 dated 06/02/2023
This is an unrestricted solicitation meaning all business size may respond. At the time of submission of offer and prior to award, the offeror shall be active in the System for Award Management (SAM) reflecting 621492 Kidney Dialysis Centers. The PSC Code is Q401 Medical Nursing.
The VA anticipates the award of one (1) Indefinite Quantity Contract (IDC) for On-Site Acute Dialysis Services to support the Sioux Falls SD VA Health Care System located at 2501 W 22nd Street, Sioux Falls SD 57105. See Performance Work Statement and Price/Cost Schedule for specific requirements and qualifications.
The period of performance is from January 1, 2024, to December 31, 2024, and includes four (4) one year option periods to be exercised at the discretion of the government. The contract is required to provide dialysis treatments on site at the Sioux Falls Hospital on an as-needed basis. Treatments are provided on an as-needed basis and is contingent upon patient needs.
Solicitation Questions. All solicitation questions shall be submitted in writing via email to Michael Cotton at michael.cotton@va.gov. Questions must be received by 11/1/2023, 11:00 AM CST.
Submission of Offer: Refer to FAR Provision 52.212-1 Addendum and 52.212-2 within the attached solicitation document for instructions on submitting an offer.
Evaluation Factors. FAR 13 Comparative Evaluation. Offerors shall submit required documentation as outlined in the solicitation.
Offer Due Date: Quotes are due to the Contracting Office by November 7, 2023, at 11:00 AM Central Time. Quotes shall be emailed to michael.cotton@va.gov prior to the solicitation closing date and time. It is the responsibility of the offeror to ensure their quote is received in full by the Contracting Officer prior to the solicitation closing date and time. Due to email file size limitations, please do not wait until the last minute to email offers.
